Abstract
In most OECD countries, healthcare delivery is facing significant challenges to provide quality care, appropriate levels of access for all and high value. In this chapter, we present a model that leverages key digital health technologies coupled with the critical components making up the social determinants of health to enable transformation around empowering individuals, leveraging the results of analytics for heightened prediction and utilising technology capabilities to design and develop superior health ecosystems that will in turn result in realising a healthcare value proposition of better access, quality and value of care. The benefits of the presented value cube are then illustrated by two case vignettes representing unique examples where a mobile solution has been designed to address a critical public health concern around diabetes self-care for vulnerable groups in the community.
